In consideration of having received (Prints, monetary compensation, digital images), I _____________ (the Model), absolutely and irrevocably authorize ______________ (the Photographer), as well as their legal representatives, assignees, and transferees to use, publish, distribute, disseminate or otherwise make publicly available for any artistic or commercial use, including advertising, publication, or illustration, images of me related to this session.
I acknowledge that by creating these images, Photographer is the sole owner of the copyright, film negatives, film positives, prints, and digital images created. Usage of the photographs may be granted to the Model and/or third parties, however the photographs will remain the property of the Photographer. I am aware that I am not entitled to any further compensation from Photographer, or any other person or organization. Permission is specifically granted for the images created to be edited, altered, distorted, used in whole or in part, in conjunction with other images, graphics, text and sound in any way whatsoever and without restrictions. I agree that the Photographer may use such photographs with or without identification of me. Photographer may choose not to use any or all images from this session at his discretion. In addition, I release Photographer from all claims for damages of any kind based on this use of said material.
I give up all claims of copyright, patent, or other ownership of the photographs, and waive the right to inspect or approve the finished product, including written or electronic copy that may accompany the photographs. Model may not further edit or alter the photographs. Any photos received from this session may be published in print or online for artistic or promotional purposes only. Model may not sell the images, or publish them for financial gain. Model agrees that should they wish to submit the photos to an online or printed publication, model must obtain written consent from Photographer before doing so. Any images published must be sourced with a proper credit to “CHD:WCK!” or “@chdwckart” on social media platforms. Images posted to any online media platforms that are not sourced, will be reported to the site and removed.
Model understands that since anyone can download an image from the Internet or make copies from printed materials, the Photographer and its assigns and transferees are not responsible for unauthorized use of the images.
Special Consideration (For Nude/Erotic photo shoots):
Model understands and agrees to the following:
- Model is at least eighteen years of age and able to provide proper identification.
- this photo session includes nudity and Model will be photographed wearing little and/or no clothing.
- the level of nudity Model shows during the photo session is at Model’s own discretion.
- Model agrees to not do anything they are uncomfortable with, and to speak up at the time if uncomfortable with a certain level of nudity or a pose.
Notwithstanding the above, the following uses are specifically excluded:

I am of full legal age and competent to contract in my own name. I have reviewed the terms above before the photo session, and I fully understand the contents, meaning, and impact of this release.
I understand that I am free to address any specific questions regarding this release, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
I am signing of my own free will, without coercion or influence upon completion of the photo session.
